Weaving two worlds together: our culture and mainstream health care

The Integrated Team Care (ITC) program supports Aboriginal and Torres Strait Islander people with chronic health conditions. It does this by overcoming barriers between primary and secondary care, physical health and social care so that people receive safe, holistic care when they need it.

Importantly, the program also provides Aboriginal health education and support to the medical community to deliver culturally appropriate care and Indigenous health services to Aboriginal and Torres Strait Islander people.

Integrated Team Care

Chronic health conditions affect more than half of Aboriginal and Torres Strait Islander people. Yet many of our people experience difficulties in understanding and managing their health conditions, navigating the healthcare system and advocating for their needs with service providers.

The Western Sydney ITC Indigenous health services program improves health outcomes for Aboriginal and Torres Strait Islander people living with chronic health conditions through access to care coordination, multidisciplinary care and self-management support.

We also work with Indigenous healthcare providers to improve their cultural competency and create greater understanding and trust between Aboriginal and Torres Strait Islander people and healthcare providers.

Culturally Safe Healthcare Support

At Western Sydney ITC, we have a deep awareness of and respect for our Aboriginal and Torres Strait Islander community, culture and connections, which enable us to build strong, genuine, trusted relationships.

We work to improve access to culturally appropriate, mainstream primary care services, including general practice, allied health and specialists, and to provide support and resources for improved self- management practices.

Importantly, the program also provides health education and support to the medical community to deliver culturally appropriate care to Aboriginal and Torres Strait Islander people.

Our Team

Led by our Health Project Officer, our team of  Care Coordinators and Outreach Workers are committed to providing our Aboriginal and Torres Strait Islander community with the care, resources and support they need to manage their health conditions.

We understand the barriers facing many in our community, and work to overcome these barriers together, through culturally appropriate care and support.

Our Care Coordinators are qualified health workers, including registered nurses and Aboriginal and Torres Strait Islander health workers, who have a deep understanding of our culture and its people.

They assist by organising services in our clients’ general practitioner care plans, arranging regular reviews by the primary care providers, providing clinical care and prioritising access to urgent and essential allied health or specialist services.

Our Outreach Workers, who have strong links to the community, help our clients with non-clinical tasks. By providing information, access and support, our Outreach Workers ensure that they empower clients with the knowledge and resources to manage their health conditions.

This includes providing information about what services are available and where they are, encouraging clients to access health care and collect medication and organising transport to and from medical appointments.
