Funded by the Western Sydney Primary Health Network (WSPHN) and led by our Health Project Officers, our team of Care Coordinators and Outreach Workers are committed to providing our Aboriginal and Torres Strait Islander community with the care, resources and support they need to manage their chronic health conditions.
We understand the barriers facing many in our community, and work to overcome these barriers together and without judgement, through culturally appropriate Indigenous health services.
